What Is a Confined-Space Entry Permit?

A confined-space permit confirms that required controls are in place for that entry.

The permit identifies the space, purpose, time, hazards, isolation, atmospheric results, ventilation, entrants, attendant, supervisor, communication, PPE, rescue, and other controls. The exact form follows the site program.

The entry supervisor checks the setup and authorizes entry. The attendant stays outside and tracks entrants and conditions. Entrants follow the permit and leave when ordered or when a prohibited condition appears.

The permit is active only for its approved time and conditions. A shift change, new task, alarm, ventilation failure, weather change, or unexpected flow may require cancellation and a new evaluation.

Post the permit where the team can use it without blocking the opening. Entries should be made as work occurs, not recreated from memory later. Cancelled permits become records that help the employer review hazards, problems, and improvements for future entries.
