Wastewater Safety · Safe Work Starts Before the Task
Why Report a Near Miss?
A near miss is a warning that caused no injury this time.
A dropped tool that lands beside a worker, a gas alarm during an unplanned opening, or a pump that starts during setup may be a near miss. The lack of injury does not make the event harmless.
Report the facts quickly. Preserve useful evidence when safe. Describe what happened, what controls were present, what changed, and what prevented injury. Avoid blame and focus on correcting the condition.
Near-miss reports can reveal weak procedures, missing training, poor equipment design, or communication gaps. Share the lesson with workers who face the same hazard. A hidden near miss cannot improve the program.
